What is Potrate 5 ER?
Potrate 5 is an extended-release tablet containing 540 mg of potassium citrate (5 mEq). It is made by Intas Pharmaceuticals and is the same medicine as Urocit-K 5 mEq in the USA.
It is not a painkiller. Potassium citrate is used to prevent kidney stones — especially calcium oxalate stones in people with low urine citrate, and uric acid stones — and to treat a kidney condition called renal tubular acidosis. It is often prescribed alongside gout treatment such as allopurinol.
How Potassium Citrate Works
Citrate in the urine binds calcium and stops calcium crystals from sticking together into stones. Potassium citrate also makes urine less acidic, which dissolves uric acid and stops new uric acid stones forming. Because the tablet releases slowly, citrate levels stay steady through the day.
How to Take Potrate 5 ER
- Take it with meals or within 30 minutes after a meal or bedtime snack.
- Swallow each tablet whole with a full glass of water — do not crush, chew or suck it.
- A common dose is 30–60 mEq a day — for example two 5 mEq tablets three times a day — but your doctor sets the dose from urine tests.
- Drink plenty of fluids — aim for at least 2.5 liters (about 10 cups) of urine a day to prevent stones.
- Blood potassium and urine citrate are checked regularly while you take it.
Side Effects
Stomach upset is the most common problem and is reduced by taking it with food.
Common and usually mild:
- Nausea, stomach discomfort or diarrhea
- Seeing the empty tablet shell in your stool — this is normal
Stop and get medical help right away if you have:
- Black or bloody stools, severe stomach pain or vomiting.
- Muscle weakness, tingling, confusion, or a slow or irregular heartbeat — signs of high potassium.
Who Should Not Take It
Do not use Potrate if you:
- Have high blood potassium or kidney failure.
- Take a potassium-sparing diuretic (spironolactone, eplerenone, amiloride, triamterene) or potassium supplements.
- Have a stomach ulcer, or a condition that slows the passage of food through the stomach or bowel.
- Have an active urinary tract infection with stones.
Talk to a doctor first if you:
- Take an ACE inhibitor or ARB for blood pressure, or an NSAID regularly — these can raise potassium.
- Have heart disease, diabetes or adrenal problems.

How to Store It
- Store at room temperature, below 86°F (30°C), away from heat, moisture and direct sunlight.
- Keep the tablets in their strip until you need one; do not keep them in the bathroom.
