What is Zyloric 100mg?
Zyloric 100mg contains 100 mg of allopurinol, the most widely used long-term treatment for gout. It is made by GlaxoSmithKline in India and is the same medicine as Zyloprim in the USA.
Allopurinol lowers uric acid, which prevents gout attacks, dissolves crystal deposits over time and helps prevent certain kidney stones. It is also used to protect the kidneys during some cancer treatments.
Understanding Gout Treatment
Gout happens when uric acid builds up in the blood and forms needle-shaped crystals in a joint — most often the big toe. A flare causes sudden, severe pain, redness and swelling. Between flares the crystals stay in the joints, and without treatment the attacks tend to become more frequent and can damage the joint.
The goal of long-term treatment is to keep blood uric acid below 6 mg/dL. At that level the crystals slowly dissolve and flares become rare. Urate-lowering medicines do not relieve the pain of a flare that is already happening — for that, doctors use an anti-inflammatory such as indomethacin or naproxen, colchicine or a steroid.
How Allopurinol Works
Uric acid is made in the body when purines break down. The final steps depend on an enzyme called xanthine oxidase. Allopurinol blocks this enzyme, so the body makes less uric acid. Levels start falling within days, but dissolving existing crystals takes months.
How to Take Zyloric 100mg
- Start low: usually 100 mg (one tablet) once a day — or 50 mg if you have kidney disease.
- Increase slowly: your doctor will raise the dose by 100 mg every 2–5 weeks, checking your uric acid, until it is below 6 mg/dL. Many people need 300 mg a day or more; the maximum is 800 mg.
- Take it after a meal with a full glass of water, and drink plenty of fluids through the day.
- Keep taking it every day, even when you have no pain.
Expect more flares at first. As uric acid falls, crystals loosen and can trigger attacks in the first few months. This does not mean the medicine is failing. Doctors usually prescribe low-dose colchicine or an anti-inflammatory for the first 3–6 months to prevent this. Keep taking your urate-lowering medicine during a flare.
A Genetic Test Some People Need First
People of Korean, Han Chinese or Thai descent, and African American patients, are more likely to carry a gene variant called HLA-B*5801, which greatly raises the risk of a severe allergic skin reaction to allopurinol. U.S. gout guidelines recommend testing for it before starting. If you carry it, febuxostat is usually used instead.
Side Effects
Most people take allopurinol for years without problems. The one side effect that matters most is a rash.
Common and usually mild:
- Gout flares in the first months of treatment
- Mild rash or itching
- Nausea or diarrhea
- Drowsiness
Stop and get medical help right away if you have:
- Any new rash, especially with fever, blisters, peeling skin, mouth sores or swollen glands — stop taking it and get medical help the same day. Allopurinol can rarely cause severe skin reactions (Stevens-Johnson syndrome, DRESS).
- Yellowing of the skin or eyes.
- Blood in the urine or pain when passing urine.
Who Should Not Take It
Do not use Zyloric if you:
- Have had a rash or allergic reaction to allopurinol.
- Take azathioprine or mercaptopurine, unless your doctor has reduced their dose — allopurinol makes them much stronger.
- Carry the HLA-B*5801 gene variant.
Talk to a doctor first if you:
- Have kidney disease — you will need a lower starting dose.
- Take warfarin, a thiazide diuretic, ampicillin or amoxicillin, or ciclosporin.
- Are pregnant or breastfeeding.
