What is Febutaz 40mg?
Febutaz 40mg contains 40 mg of febuxostat, the active ingredient in Uloric. It is made by Sun Pharmaceutical Industries Ltd and is used for long-term control of gout by lowering uric acid.
40 mg is the U.S. starting dose. If your uric acid is still above 6 mg/dL after two weeks, the dose is usually raised to 80 mg.
Sun Pharma also makes Febutaz 80mg for the higher dose.
Understanding Gout Treatment
Gout happens when uric acid builds up in the blood and forms needle-shaped crystals in a joint — most often the big toe. A flare causes sudden, severe pain, redness and swelling. Between flares the crystals stay in the joints, and without treatment the attacks tend to become more frequent and can damage the joint.
The goal of long-term treatment is to keep blood uric acid below 6 mg/dL. At that level the crystals slowly dissolve and flares become rare. Urate-lowering medicines do not relieve the pain of a flare that is already happening — for that, doctors use an anti-inflammatory such as indomethacin or naproxen, colchicine or a steroid.
How Febuxostat Works
Febuxostat blocks xanthine oxidase, the enzyme the body uses to make uric acid — the same target as allopurinol, but with a different chemical structure. It lowers uric acid strongly at standard doses and does not need a dose change for mild to moderate kidney disease.
Heart Safety
FDA boxed warning: heart risk
- In a large U.S. safety study (CARES), people with heart disease who took febuxostat had a higher risk of death from heart causes than those who took allopurinol.
- Because of this, the FDA says febuxostat should be used only by people who cannot take allopurinol or for whom allopurinol has not worked.
- Tell your doctor if you have heart disease or have had a stroke, and seek help at once for chest pain, shortness of breath, or weakness on one side of the body.
How to Take Febutaz 40mg
- Take one tablet once a day, with or without food.
- Your doctor will check your uric acid about 2 weeks after starting and adjust the dose.
- Keep taking it every day, including during a gout flare.
- The maximum dose approved in the USA is 80 mg a day.
Expect more flares at first. As uric acid falls, crystals loosen and can trigger attacks in the first few months. This does not mean the medicine is failing. Doctors usually prescribe low-dose colchicine or an anti-inflammatory for the first 3–6 months to prevent this. Keep taking your urate-lowering medicine during a flare.
Side Effects
Febuxostat is usually well tolerated apart from the early gout flares.
Common and usually mild:
- Gout flares in the first months
- Abnormal liver blood tests
- Nausea or diarrhea
- Joint pain
- Rash
Stop and get medical help right away if you have:
- Chest pain, shortness of breath, fast or irregular heartbeat, or sudden weakness or numbness.
- Yellow skin or eyes, dark urine or pain in the upper right abdomen.
- A severe rash with blisters or peeling skin.
Who Should Not Take It
Do not use Febutaz if you:
- Take azathioprine or mercaptopurine — febuxostat can raise them to toxic levels.
- Are allergic to febuxostat.
Talk to a doctor first if you:
- Have heart disease, heart failure, or have had a heart attack or stroke.
- Have liver disease.
- Have had a severe reaction to allopurinol.
- Take theophylline.
- Are pregnant or breastfeeding.
